What is "Prevention Resources and Referral Services?
Family  Resource Centers will provide quality local family support services to elible at-risk babies and their families including: outreach, information and referral services.

WarmLine has some resources for you, including:

• Playgroups in Sacramento, Placer, and Yolo. Please check our calender or call  one of our offices.

• A resource booklet developed in collaboration with Alta which contains services and resources which may be helpful. To obtain a copy, please call one of WarmLine's offices.

Who is Eligible?

• The regional center determines that an infant or toddler (birth to 3 years old) is "at-risk" for developmental disabilities following an intake and evaluation.;
• The regional center determines that a toddler between the ages of 24-35 months and has a developmental delay in one domain of 33 percent through 49 percent at the time of initial referral. The developmental domains a regional center must consider are communication, cognitive, social/emotional, self help/adaptive, and physical.; or
• Infants and toddlers (birth to 3 years old) who have a parent who has a developmental disability.

     
Who can make a referral to the Prevention Program?

Anyone can make a referral, including parents, medial care providers, neighbors, family members, foster parents and day care providers.

What is the process?

• The first step that parents take it to discuss their concerns with their health care provider.
• To make a referral, call Alta California Regional Center at 916-978-6249 (916-285-4532 for Spanish speakers) and ask to speak with an Intake Coordinator who will help you through the process to determine eligibility.
