WarmLine Family Resource Center, in partnership with UC Davis MIND Institute and Family SOUP, is enrolling participants for Mind the Gap program.

  • Participants must have a child birth to 10 years old with a suspected or recent diagnosis and live in Sacramento or Yolo county.
  • ​The Mind the Gap program offers 12 flexible peer coaching sessions (about 10 hours total) to help families learn about their child's needs and diagnosis, how to get connected with services, and reach their individualized goals.
  • WarmLine’s Peer Navigators are trained in the Mind the Gap program and bring their own expertise as parents of children with disabilities.  ​ ​
  • Medical diagnosis not necessary to participate.
